For reasons I've put in other posts, here's my summary of my guess:
1) Obamacare will cause health insurance costs to go up 20% to 30% per year in the first two or three years
2) Government will blame failure of the free market and increase subsidies to low-income people and businesses to help afford healthcare
3) Healthcare costs will continue to go up and by the end of this decade, people will propose going to a single payor system. It won't be the government that initiates it, but the people because people will be so disgusted by how the "free market" has screwed them over and will beg the government to take it over.
4) The government will say "Let's follow the European model and impose a federal VAT to pay for health insurance. This way it's fair so everyone pays and not just rich people but the rich will pay more because they will spend more." The VAT will start at 5% to 8%.
5) Healthcare costs will continue to rise under single payor and VAT will rise to 10% to 12% within a few years.
